While they can definitely get clicks, you have to remember that people recognize ads on a Google results page and often avoid them. In addition, a dentist is a highly personal service. For that reason, people tend to look at reviews, an online presence, and personal recommendations when selecting a dentist, not necessarily a slick paid advertising campaign. Not to mention, a competitor could potentially click your ads over and over to exhaust your budget and prevent them from working for you anyway (sounds petty?
